Checklist

• Parents should call the admission office at 310-288-3200 or complete the application request form to receive an application and viewbook the year before the applicant starts the 7th, 9th, or 10th grade. Each packet contains information about applying to Harvard-Westlake and the paperwork necessary to complete steps two through seven. Application packets are mailed between September and January.

• Applicants for all grades should return the response card for the Family Visiting Day with the preferred date checked. Family Visiting Days fill early. Submit your request as soon as possible. Please note that applicants for 7th grade must choose a Family Visiting Day on the Middle School Campus, and applicants for 9th – 11th grades must choose a date on the Upper School Campus.  (Please do not register for the ISEE on the same day as your Family Visiting Day.)

• Applicant Families should call the Admission Office at (310) 288-3200 to make an appointment for campus tours.

• For applicants for 7th grade, the Admission Office will contact you to schedule your weekday interview once an Application for Admission has been submitted.

• ISEE dates fill quickly!! Complete and mail the ISEE Registration Form, listing Harvard–Westlake as a score recipient. Our school code is 052135. Registration may also be completed on the ERB’s website, www.erbtest.org.  All applicants for Harvard–Westlake should take the ISEE on or before January 17, 2009.  The test may be taken at any test site.

•  Complete the Application for Admission. Include with the application:           

• A recent photograph of the applicant. (This is not required, but is strongly recommended).

• Application fee ($200.00). Please include the applicant’s name on the 
  check. This fee is waived for financial aid applicants.

• Make a copy of the completed application for your records.

• Mail the application as soon as possible in the pre-addressed envelope.

• Complete the Student Questionnaire and mail at least two weeks prior to your family’s visit and interview.
• Complete the Parent Questionnaire and mail at least two weeks prior to your family’s visit and interview.

• Print the applicant’s name on the Teacher Recommendation Forms.

• Give the forms to the applicant’s current English and math teachers with pre-addressed stamped envelopes. Students applying for the 7th grade who have only one teacher for the 6th grade should have their 6th grade teacher complete the Math Teacher Recommendation and have their 5th grade teacher complete the English Teacher Recommendation.

• Print the applicant’s name on each of these forms.

• Give the Administrative Recommendation Form and a pre-addressed stamped envelope to your child’s head of school, principal, or guidance counselor.

• Give the Authorization for Release of Applicant’s Records to an official at your child’s present school with a pre-addressed stamped envelope. Extra postage may be required. (Please do not mail this form to Harvard-Westlake.)
